It is for this reason that we emphasize the importance of social rhythm stability as one possible protective factor in individuals with a history of manic–depressive illness.

Thus, psychotherapy now is considered an integral component of treatment for individuals suffering from bipolar disorder. In general, the treatment guidelines and consensus documents recommend mood stabilizer pharmacotherapy as a first-line treatment for bipolar depression, followed by the addition of either psychotherapy or antidepressant medication. Interestingly, in most of these documents psychotherapy is marginally favored over antidepressant medication as a primary augmentation strategy, probably because of concern that antidepressant medication could induce mania whereas psychotherapy is thought to be unlikely to do so.

One thought is that this difference simply represents the typical disparity between early efficacy trials conducted in carefully selected patients at academic research centers by investigators who were passionate about the clinical care of those patients and convinced of the utility of the drug, and the “real-world” experience with more complex patients being treated by clinicians who are not necessarily specialists in the treatment of bipolar disorder. Another thought is that we have broadened our definition of what constitutes bipolar disorder in a way that includes many patients who do not have the “classic” features of those studied in the early lithium trials.
